Patient Grants

Part of the mission of the Innate Health Foundation is to provide financial assistance to people who need help paying for treatment for Irritable Bowel Syndrome (IBS) and other food allergy related health conditions.

During our first year we began fundraising and are funding patient grants as our budget allows. The board of directors has establishing an application application process. They would like to direct fully one third of the Innate Health Foundation Budget (after fundraising costs) to patient grants.

To Apply for a Patient Grant:

Please complete the Application Form. The form is a PDF file. You can type information into it and print it to sign, or print it and then fill it out. Please feel free to attach additional sheets if you need addtional space. Submit completed applications by email to contact@innatehealthfoundation.org or mail them to 1229 Madison St. Suite 1220 Seattle, WA 98104.
